The coordinator copies last week's information request because it already has the project header and familiar layout. The old reference, recipient and response date remain in the draft. A reusable format could help, but copying a finished document also imports facts and decisions that do not belong to the new question.

Keep suitable starting formats for recurring documents under the project's actual document process. Separate stable formatting and helpful prompts from information that must be established each time. A template organizes drafting; it does not turn blank filling into a complete question, confer instruction authority or guarantee a valid notice. Current facts, required content and the actual issuing and receiving process still govern the finished document.
Reuse the starting point, then check the actual document
- Choose formats the process accepts. Identify recurring documents and their actual required form, fields, references and approval or issue routes. Use the approved system or format where specified. Check who maintains it and where the current version is found. A personally convenient template should not compete with a controlled form or retain a superseded project requirement.
- Separate prompts from completed facts. Mark variable information clearly and remove old dates, recipients, quantities and conclusions from the starting copy. Keep necessary instructions or fixed text current. Obtain numbers and references through their actual arrangements rather than copying a previous identifier. Make an unanswered field visible instead of inserting a plausible value to produce a finished-looking page.
- Review the new document as a whole. Check its specific purpose, current facts, attachments, required content and receivers. Obtain the necessary review and issuing authority. Confirm that copied wording fits the actual condition and does not accidentally authorize work or imply an answer received. A correctly formatted paragraph may still describe the wrong location or conceal the real question.
- Maintain and retire starting versions. Update the template when relevant requirements, forms or procedures change, and review it at suitable points. Give users an accessible current version and handle obsolete copies through the actual record process. Preserve issued documents separately. Changing the template does not transparently correct an already issued record or justify quietly replacing its historical content.

- Copying yesterday's facts as today's default.
- Using a personal template instead of the required controlled form.

- Changing issued history by replacing it with a revised template.
