The hot-work permit: actual controls, received watch duties and authorized closure
A plumber's proposed torch work reaches a hotel ceiling cavity, but the permit names only the room and the worker. Nobody has received responsibility for the required post-work watch or monitoring. The form must connect the actual assessed controls and continuing duties before work is authorized.

Use the permit to record an actual controlled arrangement rather than manufacture permission from empty boxes. FM's August 2024 sample distinguishes authorization, named hot-work and watch personnel, post-work watch/monitoring and final checks. It is an insurer's example, not Canadian law or a universal construction form. Follow the actual jurisdiction, facility and project requirements. This lesson sets no fire-watch duration, combustible clearance, temperature test or extinguishing procedure.
Connect authorization to work and continuing responsibility
- Locate the proposed work and alternatives. State the actual task, area, timing and affected construction to the competent hot-work process. Have appropriate personnel assess whether the work can be avoided or a suitable alternative received. Identify concealed or adjoining areas needing assessment rather than treating the room number as the whole exposure. A contractor's familiar task description does not establish today's permit scope.
- Receive the required controls and authorization. Confirm that qualified personnel assessed the actual hazards and verified necessary precautions under the site's process. Retain the responsible authorized issuer and exact work limits. Equipment, protection and relevant interfaces must actually be ready, not merely ordered or checked on a form. Do not improvise covering, isolation, detector impairment or combustible removal from this lesson, or let an unsigned template serve as clearance.
- Name and receive continuing roles. Establish the required watch, monitoring, communication and emergency duties with capable people who understand their actual area and period. Retain the planned handover if responsibilities extend beyond the worker's shift. A named person already assigned conflicting work is not an available arrangement. The actual assessment and requirements determine coverage and duration; copying an insurer's sample number cannot answer every construction condition.
- Record completion through the actual final checks. Keep work completion separate from the end of required watch and monitoring. Record actual times, observations, unresolved concerns and received responsibility through the approved process. Respond to changed conditions or signs of fire using the proper emergency arrangements. Close only when the necessary checks and authorized closure have occurred; the torch being turned off or the crew leaving does not end every duty.
Common mistakes
- Treating a room number as every affected concealed area.
- Checking controls on paper before they are implemented.
- Naming a watch who is unavailable or has conflicting duties.
- Closing when the torch stops despite continuing requirements.
Checklist
A usable permit retains
- Actual task, area, assessed alternatives and scope.
- Implemented precautions and authorized work limits.
- Capable available watch/monitoring roles and communication.
- Received handovers and actual continuing periods.
- Required final checks, unresolved concerns and authorized closure.
Check your understanding
The hot-work worker is leaving, but required monitoring continues. Can the permit be closed from the finish time?
