A crane signaller repeatedly loses their place during the start-up briefing and says they have barely slept. A foreman suggests swapping them with a labourer because the lift is already booked. The immediate task decision needs a proper safety response and a competent replacement, not an informal exchange of vests.

NIOSH describes fatigue as affecting attention, reaction time, memory and judgment, with factors extending beyond one poor night's sleep. Its educational guidance calls for identifying and managing workplace contributors. A supervisor need not diagnose a cause to respond to a current concern. Apply the actual safety, health and employment processes; this lesson does not promise a particular pay outcome, assign medical fitness or authorize a replacement.
Separate the immediate work response from the personal discussion
- Address the actual task concern promptly. Use the site's safety process to protect the affected operation while responsible people determine what happens next. Identify the observed difficulty and current task rather than label the worker unreliable. Do not wait for a complete personal history when the safety concern needs action. If symptoms suggest an urgent health problem, activate the appropriate medical response.
- Speak privately and use observable facts. Explain what you noticed and invite the worker's account: you lost the sequence twice, and you have said you are exhausted. Listen without an argument about effort or a demand for private family details. Give the worker a clear receiver for health/support matters. Keep unnecessary personal information out of the crew briefing and daily production narrative.
- Receive a suitable current-work arrangement. Involve the authorized safety/management and health personnel as appropriate to determine the task response, support and relevant restrictions. Check the actual competence and authorization of any replacement; another available worker may not fill this role. Any alternate task, transport or further-work proposal needs its own appropriate decision. Coffee, willingness or a promised slower pace cannot establish suitability.
- Review the work contribution and follow through. Retain the necessary private task/action record and responsible follow-up. Bring schedule, workload or work-condition concerns into the suitable review. One reassignment is not a complete program or tomorrow's clearance. Pay and employment questions go to the proper process.
