A penetration work ticket: connect the proposed opening with required reviews
The laboratory needs new gas lines after the original service routes were coordinated. The superintendent wants each proposed opening identified and reviewed before the trade starts cutting, with the firestop response included rather than left for later.

Use an internal work ticket where the project requires it to connect a proposed penetration with the necessary decisions and follow-up. The ticket identifies the location, proposed service and responsible people; it does not itself approve a structural opening, make work safe or replace a regulatory permit. Define its purpose and authorization stages so a checked box cannot be mistaken for every required clearance.
Connect the request to completion
- Define the internal control. Agree which work requires a ticket, who requests it, who reviews the relevant subjects and who authorizes each stage. Name any separate regulatory, design, utility or safety processes. Avoid an ambiguous “approved” field that merges unlike decisions. A document controller can log the request without possessing authority to clear its technical or safety questions.
- Describe the actual proposed opening. Record the assembly, plan location, proposed services and relevant configuration information. Ask the appropriate reviewers to identify required structural, service and protection checks. Route unknown existing conditions through an assessed investigation arrangement. An opening marked on a plan is a proposal until the actual required responses establish what may proceed.
- Include the complete firestop response. Identify the applicable accepted configuration or the route to resolve it, responsible installer, work interfaces and required examination. Coordinate scope and procurement before the request disappears into another trade’s schedule. Do not promise that a standard ticket size ensures a listed system will match every future service arrangement.
- Track execution and closure. Record the authorized work, actual installed configuration, required verification and any change. Link the opening ID, photos and register entry as appropriate. Keep cutting complete, firestop installed, awaiting examination and released statuses distinguishable. Close the ticket only under the actual project criteria; a hole made successfully is not the end of the protection task.

- Calling an internal ticket a substitute for regulatory approval.
